A 58 year old male with a history of musculoskeletal pain in the left buttock/hip joint was referred for hospital assessment following failure of chiropractic treatment to alleviate the symptoms and recent worsening of the frequency and severity of the pain. The patient presented with a soft tissue mass in the left gluteal region which had previously been diagnosed and managed as a hematoma and which was now showing a gradual increase in size. The patient otherwise described himself as well but during consultation a slight cachetic appeareance was noted and it was revealed that the patient suffered more recently from a exertional fatigue and a series of recent mild infections which he dismissed as being due to a busy lifestyle and the “usual” coughs and colds. Physical examination of the patient confirmed the 10cm soft tissue mass in the left buttock region but revealed no paresthesias, numbness or weakness and full examination was otherwise unremarkable. Initial assessment was based on diagnostic imaging and further testing was requested in order for confirmation of clinical suspicions.Blood and urine tests were taken for analysis.

Perform and/or interpret the following tests as instructed during the practical session
Microbiology

• Interpret dip-slide CFU analysis of patient urine sample to determine whether or not there is a clinical infection.
• If CFU analysis does indicate clinical infection, then interpret the antibiotic susceptibility analysis of isolated organism and identify appropriate antibiotic treatment.

Biochemistry

• Total Protein

Haematology

• Bone marrow biopsy examination – Discussion during the practical.
Biochemistry
• Plasma Urea

Haematology
• Observe and report findings from the blood film.
